United Cerebral Palsy
Association of Nassau County, Inc. (UCP Nassau), is an internationally
recognized facility serving 1,700 children and adults with a wide range of
mild to severe disabilities.
SITE BASED DAY
HABILITATION Center Based Day Habilitation is
a year round program serving individuals, 21 and older, who are
developmentally disabled. Center Based Day Habilitation is located
at our 380 Washington Avenue site.
Must be enrolled in the
Waiver Program and have a Notice of Decision (N.O.D.) along with Medicaid
Service Coordination.
Comprehensive program that
focuses on skill enhancement and community integration. A variety of
club programs are offered for each consumer to choose from based on their
needs.
Occupational therapy,
physical therapy, speech therapy, psychology and social work services are
available through the Fortunoff Treatment and Rehabilitation Center.
Additional services: dental,
podiatry, gynecology, optometry, audiology, augmentative communication
laboratory, rehabilitation engineering and respite services.
