Employee Benefits

There are many benefits to working at ucpn. This Benefits Guide is designed to give you a general overview of the various benefits available to employees of ucpn. For specific details regarding any of these benefits, please speak with your supervisor or the staff in Human Resources at extension 400.

ELIGIBILITY & ENROLLMENT

Eligibility for Employees

All regular full time employee or regular part time employee scheduled to work 20 hours or more per week. Coverage for eligible employees is effective the first of the month following 90 days of employment.

Eligibility for Dependents       
The following dependents may be covered under the Health & Dental Care Program:
•    Spouse;
•    Unmarried children under the age of 19;
•    Unmarried children under the age of 23 (25 for Guardian Dental), who are full time students and dependent upon you for support;
•    Dependent children age 19 or over, who are physically or mentally disabled provided their disability began before their 19th birthday
•    Domestic Partner (requirements apply)

Enrollment

Enrollment is not automatic. Insurance carrier applications must be completed no later than 30 days from the eligibility date.  Applications & ucpn Insurance Elections Forms are available from Human Resources


EMPLOYER PAID (NON-CONTRIBUTORY) INSURANCE BENEFITS

The ucpn offers contributory (voluntary) and non-contributory (premium paid by ucpn) insurance plans. We are pleased that we can continue to offer one Medical plan, one Dental plan, Basic Life Insurance, Short Term Disability Insurance and Long Term Disability insurance (after 5 years of employment) FREE to benefit eligible employees.

Multiple insurance carriers and multiple network/plan types (HMO, Direct Access HMO, Point of Service) are available. Carriers and pricing subject to change annually. See appropriate ucpn Insurance Election Form for pricing.

Under an HMO plan you and your eligible dependents join a health care network using a designated list of physicians or a designated facility for all health care services. Direct Access HMO works the same as an HMO but does not require referrals for most in-network services. A POS plan allows you to stay in-network and pay lower co-pays or you have the flexibility to use physicians and facilities not in the network, subject to a deductible and co-insurance payments.

You may elect dental coverage through your choice of a DMO or a PPO dental plan. Under the DMO plans, you and your eligible dependents will choose a network dentist who is governed by a reduced fee schedule. Although the two DMO plans appear similar, limitations and fee schedules differ. If you choose the PPO dental plan, you can see any dentist, subject to a deductible and co-insurance payments or you can choose an in-network dentist and take advantage of lower fees subject to a deductible and co-pays.

Health Insurance Opt-Out Program
The ucpn offers an opportunity for employees to waive coverage in the Health Insurance Program, if they have coverage from another source. If you participate in the Health Insurance Opt-Out Program, you will receive a rebate, quarterly in arrears.

To ensure that no employee will be without appropriate coverage, a letter from the medical insurance carrier verifying coverage will be required. You can opt-out of health and/or dental insurance while retaining life insurance.
 
Should a life event occur, such as a spouse changing employment or losing a job, resulting in the loss of the alternative coverage, an immediate opt-back will be allowed. To be eligible, however, Human Resources must be notified of the life event within 30 days. In the absence of a life event, employees can still opt-back during any open enrollment period.

Premium Reduction Plan
Benefit eligible employees may elect to have health and dental plan coinsurance amounts reduce their taxable income, by electing this plan on the ucpn Insurance Election Form.

Basic Life & Add Insurance
Benefit eligible employees are entitled to the equivalent of a minimum of their annual salary in life insurance. In the event of an accidental death, your designated beneficiary would receive a benefit equal to twice your annual salary. The basic life insurance & ADD premium is paid in full by ucpn. Life insurance is portable/convertible with restrictions. Benefit reductions occur at age 65, 70, 75 and 80.

Dependent Care Assistance Account
The Dependent Care Assistance Account permits you to use pre-tax dollars to pay for dependent care expenses. You can elect to contribute up to $5,000 per calendar year to cover the costs of eligible day care expenses for your eligible dependents. Regular full time employees and regular part time employees, scheduled to work 20 hours or more per week are eligible to participate.

A Dependent Care Assistance Account is like a checking account. You make ‘deposits’ bi-weekly with pre-tax deductions from your salary. Then, after you incur an eligible expense, you make a ‘withdrawal’ by claiming your tax-free reimbursement from the account by submitting a bill and a claim form to the Payroll Department.

A dependent is defined as:
•    Children under age 13.
•    A physically or mentally disabled spouse, or any other person who qualifies as your dependent for federal income tax purposes, including a disabled child of any age or a dependent parent, as long as that dependent spends eight hours or more a day in your household.

If you are divorced or legally separated, you must have custody of a child for more than half of the year in order to claim reimbursement for their dependent care expenses.

Short Term Disability Benefits
The ucpn provides all employees with up to 26 weeks of New York State Short Term Disability benefits for a non-job related illness or accident. Eligibility begins four weeks after your date of hire and continues for four weeks after your termination date. Benefits begin on the 8th calendar day of absence. These benefits can provide up to 50% of your average weekly salary, up to the New York State maximum rate of $170 per week.

Long Term Disability Benefits
If you are unable to work for more than 26 weeks due to an illness or injury, you may qualify for Long Term Disability benefits. There is a 180-day waiting period for this benefit. During this waiting period New York State Short Term Disability payments are in effect. The Long Term Disability premium is paid in full by ucpn and protects 60% of your salary.




EMPLOYEE PAID (CONTRIBUTORY) INSURANCE BENEFITS


Aflac Cancer Care Protection Plan
This policy is designed to supplement your existing accident and sickness coverage only when certain losses occur as a result of the disease of cancer and other diseases and conditions caused, complicated or aggravated by or resulting from cancer or cancer treatment.

Pre-Paid Legal Services
With this coverage you can consult with your Provider Law Firm toll free by phone on any legal matter.  It provides for a free will preparation, traffic violation assistance, driver’s license recovery, personal injury and 24-hour access to an attorney in case of arrest.  The premiums are paid for by voluntary deductions from your bi-weekly paycheck.

Voluntary Life Insurance for Employees and Their Dependents
Voluntary life insurance is available and paid for by voluntary deductions from your bi-weekly paycheck. Employees may elect up to five (5) times annual salary, to a maximum of $350,000. If you have elected voluntary insurance coverage, coverage is also available for your spouse and children. Employees may elect up to 50% of their coverage (to a max of $175,000) for their spouse and up to 10% of their coverage (to a max of $10,000) for children. Dependent children are eligible age 14 days to 23 years (25 if full time student).
 
Voluntary Short Term Disability Income Coverage
This coverage provides a monthly benefit when you are sick or injured and unable to work. This six-month program is specifically designed to supplement New York State Short Term Disability benefits and/or Workers' Compensation benefits that tend to be significantly less than your full salary. This coverage is voluntary, and premiums are based upon age, salary, and waiting period. The premiums are paid for by voluntary deductions from your bi-weekly paycheck.


OTHER BENEFITS


Employee Assistance Program (EAP)
The Employee Assistance Program is a comprehensive resource designed to provide answers and advice on a wide variety of topics to help address the personal concerns and life issues of employees.  Available to you and your family members, it provides 24 hour access to professional advice and is free of charge to employees.

Tax Deferred Annuity
The TDA represents a special financial opportunity, because it allows you to defer federal income taxes as you accumulate money for your retirement through convenient payroll deduction. Enrollment can be done at any time through the Human Resources Department.  

Banking Services
Membership in Nassau Educators Federal Credit Union or Citibank will provide you with special check cashing and checking account privileges. Direct deposit is available to all employees.

Tuition Reimbursement
The ucpn offers tuition reimbursement as a benefit to regular full time employees and regular part time employees, scheduled to work 20 hours or more per week, who have successfully completed their introductory period. Eligible schools can be an accredited institution of higher education or a reputable school other than a college, including trade or vocational school. The courses must be job-related. All applications must have supervisory approval.

A limitation of two courses totaling six credit hours will be permissible during the regular school year (September through June). Three additional credit hours may be available during the summer if funds are available. Undergraduate studies will be reimbursed at 50% of tuition for a B or better and 25% for a C. Graduate studies will be reimbursed at 50% for a B or better. Pass/fail courses are eligible for reimbursement with a "Pass” being reimbursable at 50%.

Training Programs
A variety of training programs are offered by ucpn. Programs such as Defensive Driving and CPR are periodically available to all staff.

SERVICE AWARDS
The Association of ucpn presents service awards to staff for achieving significant service anniversaries (5, 10, 15, 20 years, etc.). Notification is made to all staff and awards are presented.

Vacation Time
Regular full time employees and regular part time employees, scheduled to work 20 hours or more per week, earn paid vacation. The number of vacations days and the method of earning vacations days depends upon your work location, your position and your work schedule. Consult with your supervisor for details. You are eligible to use earned vacation time after six months of employment providing you had successfully passed your introductory period.
 
Holidays
Regular full time employees and regular part time employees, scheduled to work 20 hours or more per week, earn paid holidays. The number and dates of paid holidays depends upon your work location, your position and your work schedule. Several of these holidays have been designated as floating holidays. If you choose to work on a floating holiday, you will have 90 days from the date of that holiday to take an alternate day off. Consult with your supervisor or the holiday calendar in your New Employee Orientation Handbook for details.

Personal Days
Regular full time employees and regular part time employees, scheduled to work 20 hours or more per week, earn personal days on a prorated basis depending upon their work schedule. During your first calendar year of employment, you will earn one personal day at the end of each three-month period up to a maximum of three days. After your first year, you will earn three personal days every January 1st. You are eligible to use personal days after completion of six months of employment providing you have successfully passed your introductory period.

Sick Days
Regular full time employees and regular part time employees, scheduled to work 20 hours or more per week, earn paid sick time on a prorated basis depending upon their work schedule. You will begin to earn sick time after the first 90 days of employment. Sick days, up to a maximum of 90 days, are kept in a bank and should only be used when needed for personal or family illness.

Bereavement Leave
Regular full time employees and regular part time employees, scheduled to work 20 hours or more per week  who experience the loss of an immediate family member are permitted time off with pay. See your Employee Handbook for details.

Jury Duty
Jury duty is an authorized absence. Please notify your supervisor as soon as possible if you are selected for jury duty.

On Site Child Care
Pee Wee Folks, located on our Roosevelt campus, was created to meet the needs of pre-school children ages 6 weeks to 5 years old, while their parents work in the community or at ucpn. The programs are tailored to specific age groups with emphasis placed on the particular needs of the individual child. Subsidies from the New York State Department of Social Services are available based on income eligibility. Employee Discounts
Discounts are available in Human Resources for movies, amusement parks, shows, BJ’s and Costco.

Wellness Programs & Exercise Room
The ucpn has various wellness programs and an adapted exercise room located off the Atrium. Wellness programs are announced as scheduled. The exercise room is used by our professional staff and consumers during regular business hours. Employees can sign an Exercise Equipment Waiver to use the exercise room Monday through Friday 7:00 a.m. to 8:20 a.m. and 3:40 p.m. to 7:30 p.m. Exercise Equipment Waiver forms are available in Human Resources.
